Employee-Owned Vehicles
Purpose
To set forth the claim procedure regarding losses for damage to employee owned vehicles while employees are on company premises or traveling on company business.

Procedure Statement
FINITE TECHNOLOGIES, INC. is not responsible for any loss or damage that occurs to any personal vehicles parked on or near FINITE TECHNOLOGIES, INC. premises or while being used for business purposes.
FINITE TECHNOLOGIES, INC. will reimburse an employee's personal insurance deductible, if any, up to $500 for comprehensive or collision damage that occurs while an employee is using his or her vehicle for business-related transportation, including travel between FINITE TECHNOLOGIES, INC. locations for business purposes. If the employee does not have comprehensive or collision coverage, FINITE TECHNOLOGIES, INC. will reimburse up to $500.
Commuting to and from work and using FINITE TECHNOLOGIES, INC. parking facilities near normal work stations are not considered traveling for business purposes.
Claim Procedure
When an employee files a claim for damage or loss, the following documentation will be required for reimbursement by Accounting:
1. A copy of the accident report prepared for the employee's personal insurance FINITE TECHNOLOGIES, INC. or a copy of the police report.
2. A statement signed by the employee's manager confirming that the employee was on FINITE TECHNOLOGIES, INC. business at the time of the accident and was authorized to use a personal vehicle on FINITE TECHNOLOGIES, INC. business.
3. A copy of the repair bill and the original repair receipt.
4. A copy of the declarations page of the employee's automobile insurance policy evidencing the deductible for collision damage or comprehensive coverage.
5. A properly completed and approved Expense Report. The expenditure will be charged to the employee's department number and the expense account "uninsured losses".
